Rate ceiling

Rate ceiling: How many recipients one key may accept in a single minute.

rate_ceiling_per_minute is separate from the budget period. A new key starts at 600 a minute. PATCH /limits/keys/{id} can lower it. Null does not clear it for a key: clearing a ceiling is a raise, and a raise is a person's decision.

Past the ceiling the send is refused with rate_ceiling_exceeded. That refusal carries retry_after_seconds.
